Referrals

• A referral is not required, but is a legal requirement if you want to claim a Medicare rebate.

• GP referrals are valid for 12 months, specialists 3 months.

• Having a current referral is your responsibility.

Additional services

• Radiology, pathology and hand therapy services may be used as part of Dr Ledgard’s care.

• They will attract additional fees.

• We can help provide estimates for these fees but they are your responsibility.

Collection of personal information

• Dr Ledgard collects personal information so he can offer and provide his medical services.

• He may share this information with your treating team.

• We also collect information for billing, Medicare and Health Insurance reasons.

Images

• Dr Ledgard may take photos and/or videos during consultations and surgery.

• They will only be used for documentation, teaching and research. They will not identify you.
